Instrument Self-Study - Endorsement requirement

I have decided to forgo the paid route to passing my Instrument Knowledge Test (written). I plan to use the FAA provided documentation. Then I realized a knowledge gap in my plan!

FAR 61.65:
(3) Receive and log ground training from an authorized instructor or accomplish a home-study course of training on the aeronautical knowledge areas of paragraph (b) of this section that apply to the instrument rating sought;
(4) Receive a logbook or training record endorsement from an authorized instructor certifying that the person is prepared to take the required knowledge test;

From whence do I get said endorsement once I am ready to take the test?

So, once one gets an online endorsement, where does one go and apply to take the written test? Are they scheduled at predetermined intervals or simply upon request? How long after a request is typical to get scheduled for the written?
